INSTRUCTIONS FOR SENATE
PUBLIC HEARING SELECTION
Read the instructions
below before you fill out the SENATE PUBLIC HEARING
SELECTION FORM

You must provide your own E-mail address. You
must possess your own E-mail address to register online for
the public hearings. Your
confirmation will be sent to you using that address. If you do not have an an E-mail address,
please click on "Senate Alternate
form," print it, fill it out, and
mail or fax it using the information provided.

THERE
ARE NO THIRD PARTY ONLINE REGISTRATIONS.
You may only,
register yourself or your agency using your e-mail
address.
TO
REGISTER A THIRD PARTY,
click on "Senate Alternate
form," print out the form, fill
in the information, and mail or fax it to the address
provided on the form.

Hearing Location,
Time of Day.
Please click on one of the hearing
locations, and one time slot to testify.

Required Information.
Please fill in the information requested on the
form in the fields provided (*denotes required information).
Please click on Hearing Loss Assistance if you need
help in making that selection. Then enter your choice on
the drop down menu on the form. If you make no selection
the computer will enter "none".

When you complete the
form after entering all the information, review your choices
carefully. If you find errors, you may correct them or click
on CLEAR to empty all fields and start again. When
you are sure the selections and the requested information
are correct, click on the SUBMIT button. This will send
your registration request to our office electronically for
immediate processing. Shortly after clicking on the
Submit button, you will see a "Registration Receipt Page"
appear on your screen, which will provide you with important
information as well as a short letter acknowledging receipt
of the information
you just sent us. AS SOON AS THAT PAGE APPEARS PRINT OR
SAVE THE INFORMATION FOR YOUR RECORDS.

The "Registration
Receipt Page" is Not a confirmation but
is an acknowledgement of our receipt of your request to
register. Your confirmation will follow shortly by email.

hearing confirmation.
