INSTRUCTIONS FOR COMPLETION OF LEGISLATOR'S FINANCIAL DISCLOSURE STATEMENT
(Due May 15 of each year)

INSTRUCTIONS / SAMPLE FORM

NOTE: Before completing this Disclosure Statement, please carefully read the instructions above. The completed disclosure form is due annually on May 15. Questions should be directed to Legislative Counsel at (609) 847-3901 or, if they involve the electronic filing process, to the Data Management Unit at (609) 847-3300. There are two ways to file a completed statement:

Mail or Hand Delivery: Legislators may file a completed disclosure form by mailing or hand-delivering the completed form during regular State business hours to: Joint Legislative Committee on Ethical Standards, 2nd Floor, State House Annex, Room 210, PO Box 068, Trenton, New Jersey 08625-0068. If filing by mail or hand delivery, an original signature is required on page 4; a facsimile, photocopy or stamp is not acceptable.

USE THE FOLLOWING FORMS TO FILE BY MAIL OR HAND DELIVERY WITH AN ORIGINAL SIGNATURE:

TERMINATION OR ASSUMPTION OF PUBLIC EMPLOYMENT FORM

OR

Electronic Filing: Legislators who wish to file electronically may file a completed form by emailing the document from their district office computer with a self-signed digital certificate using the forms below. If filing electronically, a self-signed digital certificate is required; a scanned image of an original signature is not acceptable.
