An Act directing the Commissioner of Insurance to establish standardized claim forms to be used by all hospital service, medical service and health service corporations, health insurers and health maintenance organizations.

    Be It Enacted by the Senate and General Assembly of the State of New Jersey:

    1. The Commissioner of Insurance shall establish standardized claim forms to be used, beginning January 1995, by all hospital service, medical service and health service corporations, all insurers authorized to write health insurance business in this State and by all health maintenance organizations authorized in this State.

    2. This act shall take effect immediately.

STATEMENT

    This bill directs the Commissioner of Insurance to establish standardized claim forms to be used by all hospital service, medical service and health service corporations, health insurers and health maintenance organizations beginning January 1, 1995.

Directs Commissioner of Insurance to establish standardized claim forms to be used by all health insurers.
