with Assembly Floor Amendments

ADOPTED: MAY 30, 1996

These floor amendments: (1) eliminate the requirement that the names of additional occupants in the vehicle at the time of the accident be included on the form; and (2) change the requirement that each "occupant" of a vehicle involved in the accident complete an abbreviated accident form to the requirement that each "operator" complete the form.
